订购美国securedservers需要填写IP Justification Form

然后需要填写一个IP Justification Form表格
不知道怎么填写
请达人们指教下，不甚感激:handshake
备注：服务器目前还没有交付给我，更不知道IP多少
信件原文Thank you for your recent order. Our provisioning staff will be releasing your server to you as quickly as possible. For additional IP space (beyond the standard /29 VLAN which is included), we are held accountable to our RIR, ARIN to collect technical justification for IP blocks larger than the initial /29.
Please fill out and send back the attached IP Justification form. Upon receipt of its accurate completion, our staff will allocate the additional IP space.
If for whatever reason you're unable to fill out this form to the minimal technical requirements, we will be providing you a refund of any additional money you have spent for additional IP space.
We sincerely appreciate your time and value your business!
